Below is a list of forms utilized by the Nichols Hills Police Department. They are available for online use or for download. Downloaded forms must be turned in at the Police Station.

 

SPECIAL TRAFFIC OPERATIONS PROGRAMS (S.T.O.P.): The patrol division will task officers with special traffic enforcement operations in response to perceived problems or citizen complaints of persistant traffic violations. We will assign an officer to observe the situation and determine if a problem exsists.

Solicitor's Permit Application can be downloaded for completion and turned in for review to the Police Department at anytime. The review process can take up to five business days. Please make your application enough in advance to allow for this.

Persons engaged in certain activities set out in Nichols Hills City policy may be exempt from being required to have a Solicitor's permit. Under the applicable circumstances, they shall first register with the chief of police. The chief of police shall then issue a registration certificate exempting them from the permit terms and conditions of the ordinance regulating persons who engage in soliciting, peddling, or selling door to door.

The “R-U-OK” program was developed out of concern for those residents of the community who are elderly or handicapped and living alone. The application can be downloaded for completion and turned in for review to the Police Department at anytime.

Please return this form to the Police Records Clerk if you need a copy of any incident, traffic collision, or arrest reports, or any other public record maintained by the Police Department. All records will be released in accordance with the terms set forth in the Oklahoma Open Records Act.
